Registration FormsDownload and print a registration form and mail it in. Participants will be selected on a "first-come, first-served" basis with priority being given to past participants. Space is limited so please contact us as soon as possible. You will be contacted with further information on finalizing the registration including how to pay the registration fee, specific details on the event, and required released forms. Please note that all indoor spaces at the Confidence Learning Center prohibit smoking. Alcohol consumption is also prohibited,

Our forms are in PDF format. Click here to download the free Acrobat PDF Reader software if needed Please DO NOT e-mail your registration forms as we must have signed original forms. Mail your completed and signed forms to the address shown on the bottom of each form. Do not combine names on a form as we must have a separate form for each person participating as an angler, attendant, boat captain or volunteer. Please be sure to tell us how many of each form you will need. Minnesota fishing licensees WILL NOT be needed to participate in this event as we are covered under the Minnesota Department of Natural Resources MinnAqua Program which exempts our anglers from the need to carry a license during our event. If you wish to continue your fishing experience after our event, you can obtain a free license if you qualify: Free permanent or annual fishing licenses for people with disabilities
